انسولین با انتقال قند موجود در خون به داخل سلولها باعث کاهش قند خون می شود ، به طور معمول تزریق ۲ بار در روز انسولین برای کنترل قندخون کافی است . اما در بعضی موارد برای کنترل بهتر قند خون جهت بعضی از افراد ۳ تا ۴ تزریق در روز نیز در نظر گرفته میشود . متأسفانه با وجود تلاشها و تحقیقات فراوانی که جهت تهیه شکل خوراکی انسولین انجام شده است هنوز انسولین خوراکی در دسترس نمی باشد و اگر چه شکل استنشاقی نیز جدیداً به بازار آمده ولی به سبب نیاز به شرایط خاص مصرف آنرا بسیار محدود است .

مصرف ناصحیح داروهای دیابت از جمله انسولین موجب پدید آمدن مشکلات فراوانی از جمله مشکلات کلیوی ، قلبی ، عروقی ، چشمی و …. برای این بیماری می گردد . لذا توجه به مصرف صحیح و منطقی داروهای در این بیماری از اهمیت فراوانی برخوردار است .

چنانچه انسولین دریافتی انسولین رگولار یا NPH میباشد لازم است انسولین نیم ساعت قبل از وعده غذایی دریافت شود . اما چنانچه از انسولین rapid – acting ( سریع الاثر ) استفاده می کنید باید بلافاصله قبل از وعده غذایی دریافت شود .

۶ نوع انسولین وجود دارد که با سرعت های مختلف فعالیت می کنند بسیاری از افراد معمولاً از مخلوط ۲ نوع انسولین استفاده می کنند عملکرد انسولین در بیماران مختلف ممکن است متفاوت باشد . هر بیمار پس از مدت کوتاهی متوجه شکل عملکرد انسولین و سرعت شروع اثر آن در بدنش می شود و بر این اساس می تواند انسولین مورد نیاز و زمان تزریق آن را به کمک پزشک خود مشخص کند .

محل تزریق انسولین نیز مهم است شما می توانید انسولین را در بازو ، ران و یا شکم تزریق کنید . اما باید به خاطر داشته باشید سرعت آزاد سازی انسولین در این مناطق با یکدیگر متفاوت است به عنوان مثال سرعت جذب انسولین از ناحیه شکمی – سریع ترین و ناحیه ران ( thigh ) آهسته ترین سرعت جذب را دارد این جذب در بازو متوسط است .

بنابراین برای تغییر محل تزریق انسولین که اغلب به علت تزریق های مکرر ضروری می شود با پزشک خود مشورت کنید .

نگهداری انسولین

چنانچه یک ویال در ظرف مدت ۳۰ روز مصرف می کنید نیاز به یخچال نمی باشد وشما می توانید انسولین را در دمای اتاق نگهداری کنید ما باقی مانده انسولین را پس از ۳۰ روز باید دور ریخته شود چنانچه یک ویال را ظرف مدت یک ماه مصرف نمی کنید آن را همیشه در یخچال نگهداری کنید بخاطر داشته باشید که همیشه یه ویال انسولین اضافه در خانه داشته باشید . انسولین را نباید خیلی داغ و یا خیلی سرد کرد منظور از دمای اتاق ۱۵-۳۰ درجه است انسولین را در فریزز و یا پشت پنجره ماشین یا منزل در معرض آفتاب مستقیم قرار ندهید .

عوارض جانبی انسولین

شامل افت قندخون ( هیپوگلیسمی ) و افزایش وزن می باشد .

افت قندخون در مصرف کنندگان انسولین در موارد زیر بیشتر اتفاق می افتد :

حذف یک وعده غذایی

مصرف بسیار کم مواد غذایی در یک وعده

انجام حرکات ورزشی بیش از حد معمول

مصرف بیش از اندازه داروی ضد دیابت

نوشیدن الکل ، بیماری ، تب و …

علائم هیپوگلیسمی

درصورت احساس تعریق ، درد ، خواب آلودگی ، گرسنگی ، عصبی شدن ، لرزش و سرگیجه ممکن است دچار هیپوگیسمی شده باشید .

تأمین قند خون در بیماران دیابتی

اگر در تست قند خون شما زیر ۷۰ باشد میتوانید برای تأمین ۱۵ گرم کربوهیدارت یکی از موارد زیر را به کار ببرید :

نصف فنجان آب میوه

یک فنجان شیر

یک یا دو قاشق چای خوری شکر و یا عسل

قند خون خود را ۱۵ دقیقه بعد چک کنید . چنانچه هنوز قند خون شما زیر ۷۰ باشد مجدداً ۱۵ گرم کربوهیدارت دریافت کنید .

چنانچه امکان تست قندخون وجود ندارد اما یکی از علائم هیپوگلایسمی وجود داشته باشد یکی از موارد ذکر شده جذب کربوهیدرات را بخورید .

چنانچه قند خون شما پایین نیست اما امکان دارد که وعده بعدی غذایی خود را حداقل تا یک ساعت بعد مصرف نکنید یک میان وعده حاوی پروتئین داشته باشید . این میان وعده میتواند شامل موارد زیر باشد :

کراکر همراه با پنیر یا کره

نصف یک ساندویج همبرگر

کراکر و یک لیوان شیر

Insulin Biosynthesis

Insulin is produced in the beta cells of the pancreatic islets. It is initially synthesized as a single-chain 86-amino-acid precursor polypeptide, preproinsulin. Subsequent proteolytic processing removes the aminoterminal signal peptide, giving rise to proinsulin. Proinsulin is structurally related to insulin-like growth factors I and II, which bind weakly to the insulin receptor. Cleavage of an internal 31-residue fragment from proinsulin generates the C peptide and the A (21 amino acids) and B (30 amino acids) chains of insulin, which are connected by disulfide bonds. The mature insulin molecule and C peptide are stored together and cosecreted from secretory granules in the beta cells. Because the C peptide is cleared more slowly than insulin, it is a useful marker of insulin secretion and allows discrimination of endogenous and exogenous sources of insulin in the evaluation of hypoglycemia. Pancreatic beta cells cosecrete islet amyloid polypeptide (IAPP) or amylin, a 37-amino-acid peptide, along with insulin. The role of IAPP in normal physiology is unclear, but it is the major component of the amyloid fibrils found in the islets of patients with type 2 diabetes, and an analogue is sometimes used in treating both type 1 and type 2 DM. Human insulin is now produced by recombinant DNA technology; structural alterations at one or more residues are useful for modifying its physical and pharmacologic characteristics

insulin action

 

Once insulin is secreted into the portal venous system, ~50% is degraded by the liver. Unextracted insulin enters the systemic circulation where it binds to receptors in target sites. Insulin binding to its receptor stimulates intrinsic tyrosine kinase activity, leading to receptor autophosphorylation and the recruitment of intracellular signaling molecules, such as insulin receptor substrates (IRS) (Fig. 2). IRS and other adaptor proteins initiate a complex cascade of phosphorylation and dephosphorylation reactions, resulting in the widespread metabolic and mitogenic effects of insulin. As an example, activation of the phosphatidylinositol-3'-kinase (PI-3-kinase) pathway stimulates translocation of glucose transporters (e.g., GLUT4) to the cell surface, an event that is crucial for glucose uptake by skeletal muscle and fat. Activation of other insulin receptor signaling pathways induces glycogen synthesis, protein synthesis, lipogenesis, and regulation of various genes in insulin-responsive cells

Glucose homeostasis reflects a balance between hepatic glucose production and peripheral glucose uptake and utilization. Insulin is the most important regulator of this metabolic equilibrium, but neural input, metabolic signals, and other hormones (e.g., glucagon) result in integrated control of glucose supply and utilization. In the fasting state, low insulin levels increase glucose production by promoting hepatic gluconeogenesis and glycogenolysis and reduce glucose uptake in insulin-sensitive tissues (skeletal muscle and fat), thereby promoting mobilization of stored precursors such as amino acids and free fatty acids (lipolysis). Glucagon, secreted by pancreatic alpha cells when blood glucose or insulin levels are low, stimulates glycogenolysis and gluconeogenesis by the liver and renal medulla. Postprandially, the glucose load elicits a rise in insulin and fall in glucagon, leading to a reversal of these processes. Insulin, an anabolic hormone, promotes the storage of carbohydrate and fat and protein synthesis. The major portion of postprandial glucose is utilized by skeletal muscle, an effect of insulin-stimulated glucose uptake. Other tissues, most notably the brain, utilize glucose in an insulin-

independent fashion
